Note:
1. All entries must be typewritten or in print.
2. Place of Delivery: UP-Manila School of Health Sciences Main Campus, Brgy. Luntad Palo, Leyte
3. Delivery shall be for a maximum period of thirty (30) calendar days.
4. Warranty shall be minimum three (3) months for supplies and materials, and one (1) year for equipment, from the date of acceptance by the procuring entity.
5. Price validity shall not be less than forty-five (45) calendar days.
6. These documents shall be attached upon submission of the quotation: (1) PHILGEPS Registration Certificate, (2) Valid Mayor-s/ Business Permit (3) Original Notarized Omnibus Sworn Statement (ABC-s above 50k), and (4) Business/ Income Tax Return (for ABC-s above 500k).
7. Bidders shall submit original brochures showing certifications of the product being offered.
8. Bidders must indicate the BRAND and MODEL NUMBER offered when appropriate.
