Description : The Isabela State University - Cauayan Campus invites registered contractors/ suppliers/bidders to submit bid proposals/price quotation of the items listed in the attached RFQ/RFP.

Upon submission of duly accomplished and signed Request for Quotation Form (RFQ), and Compliance to Technical Specifications (Annex A), the following documents shall be attached. Failure to comply/submit with any of the following requirements shall be a basis for disqualification.

a. Valid Business/Mayor-s Permit;
b. Valid PhilGEPS Registration Certificate ( for Platinum) or PhilGEPS Number (for red only);
c. Latest Income/Business Tax Return (stamped "received" by the BIR);
d. Notarized Omnibus Sworn Statement (Annex B);
e. Valid Tax Clearance Certificate;
