1. All entries must be typewritten/handwritten and all required documents shall be submitted upon submission of quotation.
2. Delivery period (7) seven working days upon receipt of purchase order.
3. Shall submit valid License to Operate (LTO) issued by FDA.
4. Price validity shall be for a period of 120 calendar days.
5. Shall submit PhilGEPS registration certificate and Mayor/Business Permit
6. Shall submit Certificate of Product Registration (CPR) and Certificate of Good Manufacturing Practice (CGMP) for drugs and medicine
7. Inclusive of Value added tax (VAT)
8. For drugs/medicines not previously used by QMMC for the past two years, supplier shall submit samples for evaluation (good for 7 patients-
full course) upon determination as the lowest bidder.
9. For I.V antimicrobials such as 2nd and 4th generation cephalosphorins, betalactam/betalactamase inhibitor and carbapenems shall be
compatible with sterile water 0.9 NACL, dextrose 5% or combination of Dextrose 5% and 0.9 NACL and stability upon dilution of 6-24 hours
