Terms of Reference:
• Service provider must be a DENR accredited Transporter, and Treater, Storage and Disposal.
• Pick-up at least 1,000 kg of medical/ healthcare waste of Gov. Celestino Gallares Memorial Medical Center, and deploy additional trips should the need arise due to an upsurge in the waste generation or as required by the hospital.
• Supply yellow plastic bags/thin plastic shrink wrap, free of charge, to be used exclusively for the hazardous waste to be collected.
• Issue Certificate of Treatment (COT) after the medical waste has been treated and disposed.
• Defend and hold harmless the hospital, its employees, or any of its staff from any claims, losses, attorney's fees, damages, liabilities, costs, expenses, or suits from injury to any person (including contractors or their employees), damage to or loss of property, or any other claims resulting from gross negligence o... - Documents:
