Description : SCOPE OF SERVICES/DELIVERABLES:
A. Pre-Survey Phase
B. Survey Implementation Phase
C. Data Consolidation, Analysis, and Report Writing
SCOPE OF WORK:
a. Finalize the sample size per defined service.
b. Inclusion of 2-3 questions in the survey instrument specifically designed to capture feedback on how services affect different client groups such as men and women, persons with disabilities, solo parents, and other relevant populations.
c. Use the ARTA-prescribed CSM questionnaire. Service specific questions can be added to the questionnaire subject to the approval of TPB.
d. Ensure that an acceptable number of respondents is met.
e. Send the CSM Questionnaire to the TPB clients and follow up the accomplishment of the form.
f. Provide tokens for ALL survey respondents (preferably digital vouchers amounting to at least PhP500.00 for external clients and PhP200.00 for internal clients per survey response).
