Kindly submit the accomplished SEALED PROPOSAL together with the following documentary requirements for Small Value Procurement (SVP):
  - Valid Mayor-s / Business Permit;
  - PhilGEPS Registration Number / Certificate;
  - Notarized NGPA Form for Omnibus Sworn Statement (Original copy)
  - Latest Income Tax Return (ITR)
