Please accomplish the attached Request for Quotations/Proposal (RFQ/P) Form for the SUPPLY AND DELIVERY OF OFFICE CHAIR AND BLOOD EXTRACTION CHAIR and indicate your confirmation on the Terms and Conditions by signing the Undertaking. Kindly submit the accomplished OPEN PROPOSAL together with the following documentary requirements for Small Value Procurement (SVP):
- Valid Mayor-s / Business Permit;
- PhilGEPS Registration Number / Certificate;
- Notarized NGPA Form for Omnibus Sworn Statement (Original copy)

The bidder shall submit on the specified date and time at the address indicated below or through online submission on or before 3:00PM, 27 AUGUST 2025. Late RFQ/P shall not be accepted.
