Please accomplish the attached Request for Quotations/Proposal (RFQ/P) Form for the SUPPLY AND DELIVERY OF SURGICAL TRAY LINES AND STERILIZATION CONTAINER PAPER FILTER and indicate your confirmation on the Terms and Conditions by signing the Undertaking. Kindly submit the accomplished SEALED PROPOSAL together with the following documentary requirements for Small Value Procurement (SVP):
- Valid Mayor-s / Business Permit;
- PhilGEPS Registration Number / Certificate;
- Notarized Omnibus Sworn Statement (Original copy)
The SEALED envelope must be duly received by the UP-PGH Purchasing Office through MANUAL SUBMISSION at the address indicated below on or before 3:00PM, 03 SEPTEMBER 2025. Late RFQ/P shall not be accepted. Submission of documents through electronic mail is NOT VALID.
