1) Deliver original/genuine items as requested;
2) Deliver the item to the DA-RFO 8, Tacloban City 15 days after receipt of P.O., otherwise a deduction of 1/10 of 1% of the contract price per day of delay shall be imposed;
3) Inform the end-user and the General Services section five (5) calendar days prior to schedule of delivery;
4) Payment shall be made after complete delivery, inspection, acceptance and submission of all necessary documents (invoice/delivery receipt, photos evidencing actual delivery, etc) required for the processing of payment;
5) Submit a snaphot of LBP account to effect payment.
