2. FOR FEEDS:
A. DURING OPENING OF BIDS, Bidder shall submit:
a1. Certificate of Analysis (CP, Moisture and Aflatoxin Test Result).
a2. BAI License to Operate if Feed Mill and/or BAI license to Operate as supplier/retailer/dealer.
B. DURING POST QUALIFICATION:
b1. BAI Certificate of Feed Product Registration.
Winning Bidder Shall:
1. Deliver the feeds not beyond two (2) months from manufacturing date.
2. Delivered feeds must be free from insects and contaminants.
3. Store the feeds in one area/warehouse and facilitate the validation test analysis of DA RFO 8 FCAL prior to delivery schedule.
4. Deliver the items by following the attached Delivery Schedule upon receipt of NTP at the San Jorge Experiment Station, San Jorge Samar
5. Inform the end-user at least seven(7) working days before the scheduled date of delivery.
