  A Winning bidder shall submit the Following;
  Deliver new/genuine items to DA-RFO 8, Kanhuraw Hill, Tacloban City;
  Deliver the items 30 Calendar days upon receipt of P.O. otherwise a deduction of 1/10 of 1% per day of delay shall be imposed:
  The winning bidder shall submit a snapshot of LBP savings account to effect payment;
  Payment shall be after full delivery, inspection and acceptance of the items.
  Submit Certificate of Registration (COR) and certificate of Product registration (CPR)
  issued by BAFS;
  Submit brochure for the offered items
  Submit Certificate of Analysis from the source
  Notify the DA-RFO8 and GSS prior to the delivery to prioritize;
  the scheduling/ conduct of inspection of the inspectorate team;
  Deliver the items to the drop-off points (see attached);
