Description : Please quote your best offer on the items/package listed below, subject to the General Conditions, using the Request for Quotation provided. CLOSED QUOTATION duly signed by you or your authorized representative may be submitted manually to the canvasser or to the address indicated above, on or before October 6, 2025 at 10:00 am.

General Conditions:
1. Please write legibly and do not leave anything blank.
2. Quotation beyond the ABC shall be automatically rejected.
3. Quotation shall be inclusive of VAT & applicable taxes
4. Price validity: 30 calendar days
5. Payment mode is by check, which shall be processed after delivery and upon submission of complete documents
6. Quotation should be duly signed by you or your authorized representative, and submitted together with:
(a) PhilGEPS Registration No.
(b) Mayor's or business permit
(c) ITR (latest)
7. Management reserves the right to reject outright any contingent bid/offer/quotation.
8. Management may require presentation of Tax Clearance at the final payment as per BIR Regulation
