Expiration Date:
1. Should there be unused stocks beyond its expiration date, supplier shall replace unopened, unused, undamaged stocks given that the unit submits a document indicating “Items for Replacement” at least 3 months before expiration date.
2. If the delivered items- expiry date is less than a year upon delivery to the unit, the supplier shall be willing to replace the items that shall expire.
Delivery Terms:
• Medical Food (Item Line No. 1-3)
o 1st delivery - 50% within 60-90 days upon issuance of Purchase Order
o 2nd delivery - 50% within 30 days upon 1st delivery
• Medical Supplements (Item Line No. 4-5)
o Full delivery within 90-120 days upon issuance of Compassionate Special Permit from FDA
