The Contractor/Supplier Shall:
1. Conduct site inspection & shall secure certificate of
Site Inspection at EFMS office
2. Secure approval of materials/equipment/work inspection
submittals before provisioning, installation of material/equipment
& commencement of any activities
3. Secure approved Work Permit and shall submit complete list of
name of workers at EFMS office prior to construction/repair activities
4. Be responsible for the replacement/restoration
on any site damage done (wether accident or
intentional) incurred during construction process
5. The Contractor/Supplier must be a valid PCAB
License holder (category: C & D, Size Range: Small B)
