  Description : The Technology Application and Promotion Institute (TAPI), through its Bids and Awards Committee, will undertake a procurement through Small Value Procurement (Sec. 34) for the following project:
  
  Total Approved Budget for the Contract (ABC): Php 210,000.00 (Inclusive of all government taxes and other applicable fees/charges)
  
  Interested suppliers are required to submit the following valid and current eligibility documentary requirements (Note: Failure to submit the following documents will result to DISQUALIFICATION):
  
  Mayor-s/Business Permit*;
  In case the Mayor-s/Business Permit is not yet available, the expired Mayor-s/Business Permit with Official Receipt for the renewal application shall be acceptable as basis for award, subject to the submission of the valid Mayor-s/Business Permit after the award of the Contract but before payment.
  PhilGEPS Registration Certificate/Registration No.;
  Notarized or Signed Omnibus Sworn Statement (OSS); and
  Duly signed Price Quotation Form (use Annex “A” ONLY).
