Description : GLASS WALL (This item includes supply, delivery and installation of 2 unit 90cm W x 2.7m H x 12mm T of tempered glass wall, frameless with frosted stickers and accessories. In addition to the scope of this item is the labor for the relocation of an existing glass partition system, currently installed at Conference Room A BCDA, BTC, composed of three (3) glass panels measuring 90 cm (W) x 2.68 m (H) each, including one (1) single swing door (all tempered-frameless glass), and to be installed at ICTD Area. Please see attached layout for reference.)

TOTAL ABC IS PHP 120,000.00

Suppliers are required to submit the following Documentary Requirements upon submission of quotation:

1) Valid Mayor's/Business Permit
2) PhilGEPS Registration No.
3) Notarized Omnibus Sworn Statement
4) Business/Income Tax Return (If the ABC is above 500k)
5) BIR Certificate of Registration (BIR Form 2303)
