Description : In accordance with Appendix A of Annex "H" (Documentary Requirements: Alternative Methods of Procurement) of the above stated Act, the supplier shall provide the following documentary requirements as a condition for award of contract:
1. Mayor-s/Business Permit
2. PhilGEPS Registration Number
3. Income Tax Return
4. Omnibus Sworn Statement

Terms and conditions:
1. Bidders shall provide the correct and accurate information required.
2. Price quotation must be valid for a period of thirty (30) calendar days from the date of submission of this quotation.
3. Price quotation must be quoted in Philippine Peso and must include the unit price and total price, inclusive of all taxes and other incidental cost to the delivery site if the contract is awarded.
4. Failure to submit all of the necessary requirements will result to rejection. Likewise, it is understood that CCWD-s specifications are the minimum requirements. The Bidder/Supplier may offer higher specifications or additional items, if any.
