Description : 1. Please refer to the attached Quotation form for your reference. Any bid submitted in excess of the approved budget will be automatically rejected.

2. Quotation must be duly received by the RBAC Secretariat Head either through manual submission or online or electronic submission at the office address indicated below, . Late quotation shall not be accepted.

3. Winning supplier must submit a certified true copy of the following documents prior to issuance of Purchase Order/Notice of Acceptance, to wit:
a. PhilGEPS Registration Number and Annex A
b. Valid and current Mayor's/Business Permit
c. Latest Income/Business Tax Return (for ABC-s above Php 500K)
d. BIR Tax Clearance

4. Delivery of the items shall be at the address indicated below.
