Description : Terms and Conditions:
1. Bidders shall provide correct and accurate information required in this form.
2. Quotations exceeding the Approved Budget for the Contract shall be rejected.
3. Subject to withholding tax.
4. Subject to 1% retention warranty if applicable (GPPB Resolution No. 30-2017, dated May 30, 2017).
5. Any interlineations or erasures shall be valid only if they are signed or initialed by duly authorized representative.
6. Bidders must be registered in PhilGEPS
7. Bidders must submit latest Mayor's Permit, DTI Permit, BIR Registration, ITR and PhilGEPS Registration Number
8. Supplier shall provide Notarized Omnibus Sworn Statement prior Notice of Award.
