Purpose: TO BE USED FOR MDRRM OFFICE/EMERGENCY OPERATION CENTER (EOC) AND EVACUATION CENTER
Please quote your lowest price for the item/s listed below not later than July 28, 2025, 10:00 A.M., subjects to the following terms and conditions, and submit your quotation duly signed by your representative by personal delivery at BAC Secretariat Office, Luna, Isabela.
TERMS AND CONDITIONS:
1. In order to be eligible for this procurement, suppliers-/service providers are required to accomplish this RFQ and submit together with the quotation on or before the deadline the following: Valid PhilGEPS Registration Certificate/Number, Valid Mayor-s/ Business permit, Tax Registration Certificate, Latest Income/Business Tax and Notarized Revised Omnibus Sworn Statement.
2. Late submission of Quotation shall not be accepted.
