proprietorship, or CDA for cooperatives; and (6) Price Quotation. All documents must be submitted
in a sealed envelope showing the control number and title of the project being quoted and must be
submitted directly to the BAC Secretariat at the City Planning and Development Office. Also,
proposal sent through facsimile will not be honored.
