  Description : Interested suppliers are required to submit the following requirements along with the quotation on or before the deadline of submission of quotations: (1) valid Mayor-s/Business Permit; (2) PhilGEPS Registration Number; (3) BIR Certificate of Registration; (4) Income/Business Tax Return; (5) Notarized Omnibus Sworn Statement; (6) Registration Certificate from SEC, Department of Trade and Industry (DTI) for sole proprietorship, or CDA for cooperatives; (7) Certificate of Product Registration form FDA of goods to be purchased; (8) Certificate of Good Manufacturing Practice, if the supplier is not an authorized distributor/dealer; (9) Batch Release Certificate (for antibiotics); and (10) Price Quotation. All documents must be submitted in a sealed envelope showing the control number and title of the project being quoted and must be submitted directly to the BAC Secretariat at the City Library Office.
