Breakdown of delivery per office:
20 gallons - Rawis Office
15 gallons - Buraguis Office
5 gallons - ICTMS Office(Embarcadero de Legazpi)
9 gallons - SLP Office(Embarcadero de Legazpi)
14 gallons - AICS Office(Pacific Mall, Legazpi City)
Requirements:
Provision of 7-10 water dispenser to be pulled out twice a month for cleaning
Monthly submission of Water Test Laboratory Certificate from DOH-accredited water testing laboratory
Submision of Sanitary Permits for the duration of the contract
Note:
Submission of Statement of Account(SOA) or Billing Statement shall be on monthly basis and pick up of check payment once available
