No. 2017-0010 dated June 23, 2014 and other
related issuances (if any).

Requirements:
1. Must be in the business for at least two (2)

2. Copy of license to operate in relation to the
provision of bottled water from the Food and
Drug Administration.
3. Must passed in Microbiological Examination.
4. Must passed in Physico-Chemical Test.
Progress Billing - every complete delivery
per month.
