Description : SUPPLY AND DELIVERY OF PURIFIED DRINKING WATER

20 liters per gallon
Inclusions:
Free use of 10 units of water dispenser
Free use of 50 brand new gallons (number of gallons may increase when needed)
Free delivery at DOH-MTRC
Note:
Supplier shall initially provide 50 filled new gallon and shall replenish empty gallons everyday until 5400 gallons are consumed.
Rationing/replenishment of empty gallon and delivery of water will occur daily, including holidays and weekends.
The pick-up of empty gallons for replenishment will occur at 8:00AM, with delivery to follow w/in 4 hours upon pick-up.
In the event of a natural or man-made emergency/calamity, the supplier shall continue to provide drinking water.
The supplier shall provide water analysis test result every quarter.
The supplier shall maintain and clean the MTRC water dispensers on the last Friday of every month.
Supplier shall provide Notarized Omnibus Sworn Statement prior Notice of Award.
