Description : REQUEST FOR QUOTATION

I. INSTRUCTIONS TO BIDDERS
"1. Bidders are required to read the instructions and fill-up the form properly.
This form will be the official canvass form in submitting bid proposal/quotation. Please use additional sheet if necessary.
2. The technical specifications must be complied with or better as applicable. Failure to comply with the mandatory requirements shall render the quotation ineligible/disqualified.
3. All goods/services to be offered must be inclusive of VAT, delivery charge, sales and other taxes, and incidental expenses. the Contract Proposal/Quotation exceeding the Approved Budget for the Contract shall be rejected.
4. Price quotation/s must be valid for a period of Forty-Five (45) calendar days from the deadline of submission.
5. Bid proposals must be supported by a brochure/manufacturer-s un-amended sales literature/unconditional statements of specification and manufacturer compliance issued by the manufacturer or appropriate document as reference for evaluation. Provide brand name for the product/s being offered.
