  1 1.00 LOT/S SUPPLY OF LABOR AND MATERIALS FOR THE TREATMENT STORAGE AND DISPOSAL OF HAZARDOUS MEDICAL WASTE MATERIALS
  Provide man power with personnel protective equipment (PPE), Loading of waste and final disposal.
  Hauling of at least 4,000 kilograms of hazardous medical waste shall be transported.
  All hazardous waste must be disposed within 90 days.
  Company must be accredited with the Environmental Management Bureau, Department of Environment and Natural Resources as transporter with treatment plant facility.
  
  Hauling will commence in 3 calendar days after the approval of Permit to Transport (PTT)
  Payment will be based on the actual number of kilograms extracted at a rate of 50.00 (fifty) per kilogram as awarded.
  Payment will be processed after the full extraction of hazardous waste.
