4. PhilGEPS Registration Certificate
5. Omnibus Sworn Statement (6th Edition)

The Supplier must submit the following documents:
1. Valid Certificate of Product Registration ( CPR)
2. License to Operate (LTO) from FDA Philippines
3. Certificate of Good Manufacturing Practices from FDA Philippines
4. If the supplier is not the manufacturer, certification from the manufacturer that the supplier is an authorized distributor
