1. supplies above stated should be delivered as specified & in good quality condition within fifteen (15) calendar days upon receipt of Notice To Proceed ( NTP).

TERMS AND CONDITION:

1. Attach to quotations/bid proposal or certificate that supplier is an authorized distributor/dealer.
2. Any defects or damage of any items found/inspected should be replaced immediately within 2 - 3 days upon receipt of notice and must have at least 2 years expiration date from delivery date.

ADDITIONAL REQUIRMENTS (TECHNICAL B)

1. Philgeps Certificate (Platinum Membership) - Updated Original download.
2. Valid Certificate of Product Registration (CPR) issued by rhe Food and Drug Administration (FDA).
3. Batch notification, Lot Release Certification from FDA or equivalent, as required by FDA for antibiotics and vaccines.
