  Description : INSTRUCTIONS TO SUPPLIERS AND SPECIFICATIONS FOR SUPPLY AND DELIVERY OF MAGNETIC SHIELD BOX ½” FOR WATER METERS
  
  1. Invitation is restricted to Filipino Citizens/Sole Proprietorships, Cooperatives and Partnerships or Organizations with at least Sixty Percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines.
  
  2. Prospective Suppliers shall submit their duly signed quotation together with the authentic copy of the original required documents in SEALED ENVELOPES in three copies.
  
  3. The required documents refers to the following;
  a. Mayor-s/Business Permit
  b. PhilGEPS Registration Number
  c. Income/Business Tax Return
  d. Omnibus Sworn Statement
  e. Warranty of one (1) year (if awarded the contract)
  f. Authority of the Signatory to transact and enter into a contract with the Mangaldan Water District
  g. Sample Subject Material
  
  4. Failure to submit one or more of the required documents, the BAC shall rate the supplier as “failed”.
