3. Properly sealed and labeled kits/vial indicating specific
reagents, quantity and must be free from any defect and
conforms to product specifications
4. Authenticated copy of certfifcation/ agreement from the
Manufacturer/principal of notarized copy of
certification/agreement from the subsidiary office in the
Philippines that the PROSPECTIVE BIDDER
