c. Income/Business Tax Return (latest available)
d. Omnibus Sworn Statement
e. BIR Certificate of Registration (COR) indicating a line of business related to the supply/distribution of laboratory or scientific equipment
f. If applicable, FDA-issued Certificate of Product Registration (CPR) or Certificate of Medical Device Registration (CMDR)
