Description : Interested suppliers are required to submit the following requirements; (1) Registration certificate from SEC or DTI for sole proprietorship or CDA for cooperatives, (2) Valid/current Mayor-s Permit, (3) Certificate of PhilGEPS Registration Platinum Membership, (4) Tax Clearance Certificate, (5)Audited Financial Statements stamped “received by” the BIR of its duly accredited and authorized institutions, for the proceeding calendar year, (6) Statement of all its ongoing and completed government and private contracts with in two (2) years from submission of quote (7) Bid Security, (8) Conformity with Technical Specifications, (9) Duly signed After Sales services/parts (Warranty), (10) Omnibus Sworn Statement (OSS), (13) Net Financial Contracting Capacity (NFCC) and (14) Price Quotation in a sealed envelope. All quotations must be submitted in a sealed envelope showing the control number and name of title of the project being quoted and must be submitted directly to the BAC Secretariat at the City Library Office. Also, no fax or email quotation will be honored.
