TERMS AND CONDITIONS:

1. The expiration date of the item/s to be delivered must not be less than 24 months. In case items with an expiration date of less than 24 months are delivered, the supplier must provide a guarantee letter agreeing to replace any near-expiring items 4 months before their actual expiration date.

2. Partial delivery is SUBJECT TO PRIOR APPROVAL.

3. The quoted price must include all costs incidental to the delivery of the items from the supplier-s warehouse to Sulu Sanitarium and General Hospital.

Prospective Bidders shall submit the following requirements together with the accomplished RFQ:
