Description : REQUEST FOR QUOTATION

The Local Government Unit of Santa Praxedes, through its Municipal Development Fund, will undertake a Negotiated Procurement-Small Value Procurement for the “Supply and Delivery of Costume” in accordance with Section 53.9 of the implementing Rules and Regulations of Republic Act No. 9184.

Interested suppliers are required to submit their valid and current Mayor-s/Business Permit or its equivalent, Tax Clearance, DTI Certificate, SEC or CDA Certificate, Latest Audited Financial Statements (Business Income Tax Return), or PhilGEPS Certificate of Registration (Platinum), and price quotation form (Annex “A”).

Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated in Annex “A” and other terms and conditions stated in the price quotation form.
