  Instructions:
  1.) Download the OFFICIAL BID FORM (Request for Quotation) at PhilGEPS or get a copy at the Bids and Awards Committee (BAC) prior to the submission of bid quotation.
  2.) Quotation/s must include all taxes.
  3.) Quotation/Proposal shall be submitted to the Chairman, BAC, National Irrigation Administration Davao Oriental - Irrrigation Management Office, Bagumbayan, Lupon, Davao Oriental.
  4.) Only sealed canvasses shall be accepted.
  5.) The Bidder is required to affix his/her signature across the flap of the sealed envelope.
  6.) Technical Specifications for the items to be bid are detailed in the Request for Quotation Form.
