In line with the above-mentioned procurement, kindly quote your lowest price on the item/s listed on the attached Quotation Form (Annex A). Please submit your quotation duly signed by the owner or your authorized representative to the Office of the BAC Secretariat, Municipal Hall, Purok 5, Barangay Lublub, Alfonso Castañeda, Nueva Vizcaya subject to the General Conditions specified below.
