NOTE:

1. Certificate of Exclusive Distributorship/Dealership from the Manufacturer must be submitted.

2. Certification of Chemical Analysis from the Product Manufacturer must be submitted.

3. Report of Analysis from Philippine Institute of Pure and Applied Chemistry (PIPAC).

4. Chlorine should be in a sealed plastic container with Materials Safety Data Sheet (MSDS).

5. Chlorine must be NSF Certified.

6. As a government agency, SWD shall deal only with legitimate suppliers/ contractors which issue BIR registered official receipts.

7. Bidder/Supplier should have office and warehouse in Bacolod City or Negros Occidental. Complete with pictures of the facilities. Should be in existence for at least Five (5) years.

8. Delivery period from Notice to Proceed is Fifteen (15) Calendar days.br...
