Description : TERMS AND CONDITIONS:
1. All entries must be typewritten or legibly written.
2. Delivery/Performance period shall be within three (3) working days upon receipt of Notice of Award/Purchase Order (PO).
3. Administrative penalties pursuant to Section 69 of the Revised IRR-RA 9184 shall be imposed for non-delivery without valid reason.
4. Price validity shall be for a period of 120 calendar days.
5. PhilGEPS Registration, Suppliers Registration Certificate, Mayor-s Permit, and DTI Registration shall be attached upon submission of the quotation.
6. Bidders shall submit certification of the product complying to the required standard as enumerated in the RFQ
7. All required Philippine Taxes must be included in the bid proposal.
8. Incomplete submission of the required documents is a ground for bid proposal rejection.
9. The awarding of this RFQ will be on a lump sum basis. Prospective suppliers must quote for all items. Otherwise they will be subjected for disqualification.
