SUPPLY AND DELIVERY OF 5 AMPS AMIODARONE AND (11) OTHER ITEMS UNDER PURCHASE REQUEST NO. 1000-25

1. The Provincial Government of Misamis Oriental, through the MOPH-Magsaysay-Drugs and Medicines intends to apply the sum of One Million Three Hundred Two Thousand Three Hundred Twelve Pesos and Fifty Centavos (Php1,302,312.50) being the ABC to payments under the contract for the Supply and Delivery of the following:

1 Amiodarone 50mg/mL, 3mL 5 amp
2 Budesonide nebule 350 neb
3 Calcium Gluconate 100mg/mL, 10mL 50 amp
4 Ceftriaxone Na 1G 2000 vial
5 Ciprofloxacin 2mg/mL, 100mL 100 vial
6 Clindamycin 150mg/ml, 4ml 100 amp
7 Epinephrine 1mg/mL, 1mL 100 amp
8 Furosemide 10mg/mL, 2mL 300 amp
9 Gentamicin 40mg/mL, 2mL 300 amp
10 Lidocaine 2%, 1.8mL 750 carp.
11 Metronidazole 5mg/mL, 100mL 200 vial
12 Sterile Water for Injection, 100mL 300 bt

Bids received in excess of the ABC shall be automatically rejected at bid opening
