Description : COVID-19 Rapid Antigen Kit

Terms of Reference:
1. Expiration date of reagents must be one (1) year or longer from the date of delivery. 2. Staggered delivery of reagents in three (6) months or as per end user's request as the need arise. 3. Initial delivery should be within 7-10 days upon receipt of Purchase Order (PO).
4. Valid and current Certificate of Product Registration (CPR) or issued by Philippine Food and Drug Administration (PFDA).
5. Request for replacement of any reagent or supplies that are deemed unacceptable by the end user, after a thorough evaluation, without any additional cost.
6. Transact all deliveries during office hours except for emergency deliveries which shall follow proper protocol in this regard.
7. Labelling Instructions: Each box and /or carton should be imprinted or stickered with non-removable or permanent sticker or label that is binding, and with residue and tearing, if removed:
