1. The Provincial Government of Misamis Oriental, through the MOPH-Initao-Drugs & Medicines intends to apply the sum of One Million One Hundred Ninety-Five Thousand Nine Hundred Thirty-Five Pesos (Php1,195,935.00) being the ABC to payments under the contract for the Supply and Delivery of the following:

1. omeprazole 40mg/ml 1000 vials
2. paracetamol 150mg/2ml 300 amps
3. phytomenadione 10mg/ml 300 amps
4. potassium chloride 2meq/ml, 20ml 200 amps
5. propofol 10mh/ml, 2ml 5 amps
6. ranitidine 25mg/ml, 2ml 500 amps
7. salbutamol 1mg/ml, 2.5ml 1500 nebs
8. salbutamol + ipratropium 2.5mg/250mcg/ml 2000 nebs
9. salmeterol + fluticasone 25mcg/250mcg 50 MDI
