Terms of Reference:
1. Staggered delivery of supplies for six (6) months or as per user-s request as the need arise.
2. Expiration date must be one (1) year or longer from the date of delivery.
3. Initial delivery should be within 7 days upon receipt of Purchase Order (P.O.).
4. Labeling Instructions: Each box and/or carton should be imprinted or stickered with non-removable or permanent sticker or label that is binding, and with residue and tearing, if removed:
