The Presidential Management Staff (PMS), through Administrative Service - Procurement Management Division (AS-PMD), intends to procure subscription to warranty and maintenance services for the APC symmetra 48KVA and smart uninterruptible power supplies (UPS) (see attached TOR) with an Approved Budget for the Contract amounting to Nine Hundred Thirty-Five Thousand Pesos (PhP935,000.00) through Section 34 (Small Value Procurement) of the Implementing Rules and Regulations of Republic Act (R.A.) No. 12009.
Please submit your duly signed quotation form including the Terms and Conditions (Annex A) together with the following documents not later than 12 January 2026 at 2:00 PM:
a. PhilGEPS Registration Number;
b. Mayor-s/Business Permit;
c. Income/Business Tax Return ;
d. Notarized Omnibus Sworn Statement and if applicable, Original Notarized Secretary-s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney .
