  1. All entries must be typewritten/handwritten and all required documents shall be submitted upon submission of quotation.
  2. Delivery period (7) seven working days upon receipt of purchase order.
  3. Shall submit valid License to Operate (LTO) issued by FDA.
  4. Price validity shall be for a period of 120 calendar days.
  5. Shall submit PhilGEPS registration certificate and Mayor/Business Permit
  6. Shall submit Certificate of Product Registration (CPR) and Certificate of Good Manufacturing Practice (CGMP) for drugs and medicine
