The Presidential Management Staff (PMS), through Administrative Service
Procurement Management Division (AS-PMD), intends to procure Security Services for
the PMS Northern Luzon Field Unit (NLFU) - La Trinidad, Benguet (see attached TOR)
with an Approved Budget for the Contract amounting to Nine Hundred Twenty Six
Thousand Pesos (PhP926,000.00) through Section 34 (Small Value Procurement) of the
lmplementing Rules and Regulations of Republic Act (R.A.) No. 12009.

Please submit your duly signed quotation form including the Terms and Conditions
(Annex "A") together with the following documents not later than 19 December 2025 at
2:00 PM:

a. PhiIGEPS Registration Number;
b. Mayor's/Business Permit;
c. lncome/Business Tax Returnl;
d. Notarized Omnibus Sworn Statement2 and if applicable, Original Notarized
Secretary's Certificate in case of a corporation, partnership, or cooperative; or
Original Special Power of Attorney.
