Description : 1 1.00 L.S ENVIRONMENTAL/SANITARY SERVICES
SUPPLY OF LABORANDMATERIALS FOR
THETREATMENTSTORAGEANDDISPOSAL
OF HAZARDOUSMEDICALWASTE
MATERIALS.
INCLUSIONS:
1. Provide manpower with the personnel
protective equipment (PPE). Loading of waste
and final disposal.
2. Hauling of at least 3, 163 kilograms of
hazardous medical waste shall be transported.
3. All hazardous waste must be disposal within
90 days.
4. Company must be accredited with the
Environment Management Bureau, Department
of Environment and Natural Resources as
transporter with treatment plant facility.
Hauling will commence in 3 calendar days after
the approval of Permit to Transport (PTT).
Payment will be based on the actual number of
kilogram extracted at a rate of 30.00 (Thirty)
pesos per kilogram as awarded.
Payment will be processed after the full
extraction of hazardous waste.
The supplier shall notify the PGSO a day before
the actual collection and transport of hazardous
medical waste.
