Please accomplish the attached Request for Quotations/Proposal (RFQ/P) Form for the SUPPLY AND DELIVERY OF VARIOUS HOUSEKEEPING SUPPLIES (BATTERIES) and indicate your confirmation on the Terms and Conditions by signing the Undertaking. Kindly submit the accomplished SEALED QUOTATION together with the following documentary requirements for Shopping:
- Valid Mayor-s / Business Permit;
- PhilGEPS Registration Number / Certificate;

The SEALED envelope must be duly received by the UP-PGH Purchasing Office through MANUAL SUBMISSION at the address indicated below on or before 3:00PM, 12 FEBRUARY 2025. Late RFQ/P shall not be accepted. Submission of documents through electronic mail is NOT VALID.

The envelope shall be marked / contained of the following:
- Name and address of the prospective supplier
- Name of the Title Project to be bid/quote
