3. Price quotation/s to be denominated in Philippine Peso shall include all taxes, duties, and/or levies payable.

5. Quotations exceeding the Approved Budget for the Contract shall be rejected. Please attach the following requirements. Please see the additional list of requirements at the back.

b) Sworn Affidavit of the bidder that it is not related to the Head of Procuring Entity. Please see additional requirements at the back.
Terms and Condition:
1. Price quotation/s shall be valid for a period of at least sixty (60) calendar days from the date of submission.
2. Delivery period within thirty (30) calendar days from receipt of the Approved Purchase Order.
