NOTICE TO ALL PROVIDERS
The National Privacy Commission intends to procure the SUPPLY AND DELIVERY OF DRINKING WATER (APP Item No. 2025-0017). As such, providers or suppliers of known qualifications are hereby invited to submit their quotations/price proposals signed by their authorized representative, the details of which is discussed in the attached Technical Specifications not later than 05:00 PM., 11 June 2025.
The service providers/suppliers must also submit the following requirements:
1.Copy of the current Mayor-s/Business Permit issued by the city or municipality where the principal place of business of the prospective bidder is located;
2.Current PhilGEPS Registration Number;
3.Notarized Omnibus Sworn Statement with applicable attachments1;
4.Microbiological test results from duly-licensed/DOH accredited Environment Health Laboratory Service to provide that the drinking water conforms/compliant with the acceptable standards;
5.Sanitary Permit/Certificate of Potability for the drinking water; and
